Corona-insônia: o fenômeno que está impedindo as pessoas de dormir na pandemia

A pandemia de coronavírus teve um efeito profundo na rotina das pessoas e tornou mais difícil dormir para muitas pessoas — a tal ponto que especialistas criaram até um termo para isso em inglês: coronasomnia. Em português seria algo como “corona-insônia” ou “covid-insônia“.

É um fenômeno que atinge pessoas no mundo todo: insônia associada ao aumento do estresse por causa da pandemia de covid-19.

No Reino Unido, um estudo de agosto de 2020 da Universidade de Southampton mostrou que o número de pessoas com insônia aumentou de uma em seis para uma em quatro, com mais problemas em alguns grupos, incluindo mães e trabalhadores essenciais.

Na China, as taxas de insônia aumentaram de 14,6% para 20% durante o período de isolamento social. Uma “prevalência alarmante” de insônia clínica foi observada na Itália. Na Grécia, quase 40% dos entrevistados em um estudo de maio disseram estar sofrendo problemas para dormir. A palavra “insônia” foi mais pesquisada no Google em 2020 do que nos anos anteriores.

Entramos no segundo ano de pandemia, e meses de distanciamento social abalaram rotinas diárias, apagaram os limites da vida profissional e profissional e trouxeram incertezas para as vidas de todos. Isso tudo trouxe consequências desastrosas para o sono, e a saúde e a produtividade podem enfrentar sérios problemas por causa disso.

A insuficiência de sono — que muitas autoridades de saúde classificam como menos de sete horas por noite — também afeta o trabalho. Muitos estudos demonstraram que isso aumenta a probabilidade da pessoa cometer erros, prejudica a concentração, aumenta o tempo de reação e afeta o humor.

O fato de tantas pessoas estarem atualmente com insônia tem com certeza a ver com as circunstâncias desafiadoras, “quase bíblicas“, da pandemia, afirma o psiquiatra e neurologista americano Steven Altchuler, especializado em medicina do sono.

Diversos fatores foram responsáveis por essa queda na qualidade do sono.

Primeiro, as rotinas diárias das pessoas foram interrompidas e seus ambientes, alterados, tornando difícil manter o ritmo circadiano intacto. Normalmente, nossos dias seguem uma rotina de horários para acordar, se locomover de um lugar para o outro, fazer intervalos no trabalho e dormir. Mas a covid-19 bagunçou tudo isso.

“(Com o trabalho remoto) perdemos muitos dos elementos externos que regulam nossa rotina, como as reuniões no escritório, o horário do ônibus, o intervalo para o almoço“, diz Altchuler.

“Seu cérebro está condicionado: em certos horários você está no local de trabalho, em outros está em casa relaxando. Existe uma diferenciação aí. Agora, estamos em casa o tempo todo“, diz Angela Drake, professora de saúde clínica na Universidade da Califórnia, que trata de pacientes com distúrbios do sono. Ela também destaca o fato de que, quando trabalhamos em casa, podemos fazer menos exercícios e ter menos exposição à luz natural — duas coisas que contribuem para um sono melhor.

Há também a questão do desempenho no trabalho. O desemprego em muitos países é o maior dos últimos anos, então não é surpresa que quem está empregado queira trabalhar duro para manter seu empregos.

O problema é que trabalhar em casa pode confundir certos limites, com muitas pessoas relatando trabalhar mais tempo ou horários irregulares. “Nossa tendência é ter limites muito menos claros entre casa e trabalho“, diz Altchuler. “As pessoas tendem a ficar acordadas até mais tarde.“

Para muitas pessoas, deixar “o trabalho no trabalho” agora é completamente impossível, e desconectar-se do estresse diário é mais difícil do que nunca.

Em dezembro, a Universidade de Ottawa analisou 55 estudos globais com mais de 190 mil participantes para medir a proeminência da insônia, da depressão, da ansiedade e do transtorno do estresse pós-traumático (TSPT) desde o início da pandemia. Todos os distúrbios aumentaram em pelo menos 15% entre os profissionais de saúde, com a insônia tendo o maior aumento, de quase 24%.

Altchuler aponta que a insônia é “comumente associada ao TSPT” e, se você é um trabalhador da linha de frente ou não, é comum que a insônia apareça após grandes eventos mundiais negativos.

Em geral, sempre que alguém passa por um trauma — seja uma crise de saúde generalizada como a covid-19, um desastre público como o 11 de setembro ou algo mais individual como um acidente de carro — é possível ter problemas de sono persistentes que vêm junto com o TSPT.

O que podemos fazer sobre isso?

Os especialistas dizem que é importante procurar ajuda quando os problemas de sono persistem — especialmente hoje em dia.

“Como a pandemia continuou por um período significativo de tempo, não apenas alguns meses, há uma grande possibilidade de que as taxas de insônia não caiam“, diz Lisa Artis, vice-CEO da Sleep Charity, uma ONG que promove saúde do sono no Reino Unido.

“Se as pessoas não procuram ajuda quando começam a sofrer com a falta de sono, é provável que seus problemas de sono se tornem um distúrbio permanente do sono, ou seja, insônia, e infelizmente não há uma solução rápida… É difícil quebrar hábitos formados“, diz Artis.

Mas há boas notícias. Altchuler destaca alguns avanços, como a “rápida expansão da telemedicina” durante a quarentena. Isso pode ter um impacto positivo já que muitas pessoas não têm condições ou têm relutância de visitar clínicas médicas pessoalmente.

O tratamento mais comum para problemas de sono é a terapia cognitivo-comportamental para insônia, que melhora a chamada “higiene do sono” — uma série de atitudes que podem ser tomadas antes de dormir para evitar problemas, como não fumar nem beber antes de dormir, apagar todas as luzes, etc.

A terapia também treina o cérebro para associar a cama com o sono através de mudanças comportamentais, como não trabalhar na cama e não ver televisão no quarto.

Um estudo da Universidade de Michigan do ano passado mostrou que os pacientes que buscaram a terapia por meio da telemedicina receberam um tratamento tão eficaz quanto o teriam recebido pessoalmente, o que pode possibilitar um acesso mais amplo à assistência médica.

Existem também pequenas coisas que as pessoas podem fazer para melhorar a higiene do sono sem fazer a terapia.

Os médicos aconselham limitar o consumo de notícias para evitar a ansiedade não usar o telefone celular como despertador — ele é outro item associado ao trabalho e muitos emitem uma luzinha mesmo quando estão com a tela desligada, o que é ruim para a produção de melatonina (o hormônio que induz o sono) no corpo.

Também é aconselhado manter o celular em outro cômodo quando for dormir e ter um relógio despertador na mesa de cabeceira, mas não ficar olhando para ele enquanto tenta adormecer.

Os médicos lembram que as circunstâncias atuais estão longe de ser normais — então não é surpresa que estejamos enfrentando desafios. “A última vez que houve esse tipo de evento foi há mais de 100 anos“, diz Drake. “Nenhum de nós teve que lidar com esse tipo de coisa antes.”
