Depressão não é tudo igual: conheça os tipos menos comuns e que impactam na qualidade de vida

A pandemia de covid-19 tem posto em risco não só a saúde física, mas mental da população. O isolamento imposto pelas medidas para conter a propagação da doença, a crise econômica, o desemprego e a própria ansiedade em relação à contaminação são alguns fatores que colocam à prova nosso bem-estar psicológico.

Não é à toa que estudos mostram um aumento nos índices de depressão, doença que atinge mais de 320 milhões de pessoas no mundo, de acordo com a Organização Mundial da Saúde (OMS).

A depressão que impacta a maioria dos pacientes é a unipolar, também conhecida como transtorno depressivo maior. A causa mais comum é de cunho genético, mas também pode ser provocada por perdas, estresse e até problemas neurológicos.

Mas existem classes diferentes de depressão. De acordo com a psiquiatra Ana Paula Carvalho, coordenadora da Liga de Depressão do Instituto de Psiquiatria do Hospital das Clínicas da Faculdade de Medicina da USP (FMUSP), há três tipos menos conhecidos e comuns na população: depressão psicótica, distima e bipolar.

“No caso dessas doenças, muitas vezes o diagnóstico deixa de ser feito pelo próprio psiquiatra ao longo dos anos, passando despercebido e sem uma avaliação criteriosa. Na depressão bipolar, por exemplo, a descoberta pode levar mais de dez anos, dificultando o tratamento”, diz a psiquiatra.

Por serem mais difíceis de diagnosticar, é importante conhecer melhor cada uma.

Depressão bipolar: a mais difícil de ser identificada

O transtorno bipolar do tipo 1 é a forma mais clássica e é caracterizado pela euforia (mania e hipomania). Já o do tipo 2, que é a depressão bipolar, o paciente apresenta quadros de tristeza e hipomania — estado mais leve de euforia, otimismo e, às vezes, agressividade.

“Geralmente, quando ocorre somente a bipolaridade é mais fácil de reconhecer a doença, já que o paciente apresenta sintomas evidentes. Porém, quando o quadro depressivo aparece em conjunto, pode levar anos até chegar a um diagnóstico preciso”, afirma Luiz Dickeman, psiquiatra da Universidade Federal de São Paulo (Unifesp).

O especialista explica que, para o paciente ser caracterizado com a condição, ele precisa ter episódios de hipomania pelo menos uma vez ao longo da vida, durante quatro dias ou mais.

“Ele deve ser expor a atividades de risco como gastos excessivos, vontade exacerbada de fazer sexo, pouca horas de sono.”

Um estudo publicado na revista Brasileira de Psiquiatria mostrou que, em média, leva-se oito anos para diagnosticar um paciente com depressão bipolar.

Outras publicações americanas já mostraram que o diagnóstico pode demorar até 15 anos.

“O erro ocorre pelo próprio médico, que não investiga os sintomas a fundo e acredita que a pessoa sofra somente com a unipolar, que é a mais comum e conhecida. Além disso, o paciente omite se já teve episódios de hipomania em anos, aumentando, muitas vezes, o risco de suicídio”, diz Dickeman, que também é diretor do Instituto Brasileiro de Farmacologia Prática (BIPP).

Fabiano Alves Gomes, psiquiatra e professor da Queen’s University, no Canadá, explica que, enquanto a prevalência da depressão unipolar é de aproximadamente 15%, as formas mais clássicas do transtorno bipolar (tipo 1 e tipo 2) têm prevalência em cerca de 3% da população.

Neste tipo, o tratamento mais indicado não é com antidepressivos, e sim, com estabilizadores de humor. “É possível atingir recuperação completa principalmente se tratada adequadamente e no início”, afirma Gomes.

Distimia: menos conhecida entre os próprios pacientes

O transtorno depressivo persistente ou distimia é uma depressão crônica, caracterizada por sintomas que duram por até dois anos ou mais. A causa ainda é pouco conhecida pelos médicos, mas os especialistas acreditam que seja multifatorial.

Ela é menos comum, e o próprio paciente pode não reconhecê-la por achar que os sinais estão relacionados à personalidade.

“É a típica pessoa que reclama toda hora, que tem uma visão pessimista das coisas e vive em uma rotina de lamentações. O que dificulta o diagnóstico é que na grande maioria dos casos, familiares e amigos acham que é o ‘jeito’ dela e que vai passar com a idade”, afirma Carvalho.

No entanto, os sintomas podem evoluir para uma depressão mais grave. O paciente demora a procurar ajuda porque acredita que não é nada e o quadro depressivo só piora.

“Às vezes, permanece com o problema por 20 anos, até ir ao médico. O distêmico está abaixo da linha da normalidade”, reforça a especialista.

Diferentemente da unipolar, na qual os neurotransmissores são afetados, a distimia não altera as funções biológicas do paciente.

“É uma pessoa funcional, que come, dorme, consegue trabalhar. O grande problema é que a doença provoca um impacto bem grande na qualidade de vida, já que o indivíduo reclama o tempo todo, está sempre de mau humor e sofre com baixa autoestima”, diz Dickeman.

O tratamento mais indicado é a combinação de medicamentos em doses geralmente mais altas do que os da depressão unipolar, além de psicoterapia.

Depressão psicótica: a forma mais grave do transtorno

Além de tristeza, o paciente sempre apresenta sintomas psicóticos como alucinações e delírios. “É uma alteração dos cinco sentidos. Você pode ouvir e ver coisas, sentir cheiros e até toques na pele”, explica Dickeman.

Considerada rara pelos médicos, a depressão do tipo psicótica é provocada por luto, traumas ou cobrança excessiva em relação a si mesmo.

Segundo o psiquiatra da Unifesp, ela é mais fácil de diagnosticar, já que os sintomas são percebidos nos primeiros atendimentos. Porém, o que acontece muitas vezes, é que a doença causa um desgaste familiar muito grande, e o paciente fica fora de si com frequência.

“Quase não é conhecida pelas pessoas e acaba surpreendendo todos que convivem, porque os sintomas são exacerbados. Às vezes, os sintomas são confundidos com os da esquizofrenia”, diz.

Neste quadro depressivo, as chances de reações suicidas são maiores e, por isso, familiares devem ficar atentos. É necessário ter um acompanhamento médico frequente até a melhora dos sintomas.

“É fundamental distinguir os vários tipos de depressão, pois o tratamento é diferente. A depressão psicótica exige associação de antipsicóticos.
